Billing 

We request payment at the time services are rendered. We accept cash, checks, VISA and MasterCard. Certain surgical procedures may require advance payment or deposit. We do not participate with any insurance plans. You will be provided with a claim form to submit to your insurance company for reimbursement. Call your insurance carrier for claim submission instructions. Under certain circumstances additional information may be necessary to process your claim. You are responsible for payment in full regardless of any insurance company's determination of usual and customary fees. 

Appointment Reminders

Donna Hagberg MD LLC sends out reminders for annual visits by both mail and telephone, as well as periodic reminders for follow-up appointments for short term medical conditions that require close observation. As a courtesy, all patients are called at least 24 hours prior to their scheduled visit on either their home or cell phone number of record. Patients are responsible for keeping their appointments regardless of the office's notification policies. 

Cancellation Policy

Patients will be charged a fee of $100.00 if an appointment is missed unless the office is notified 24 hours prior to the scheduled appointment or there are mitigating circumstances. This policy is necessary so other patients are not denied an appointment and out of respect for the doctor's time. Cancellation policies are posted by the business office and in the reception area. Patients will also be informed at the time of their telephone reminder calls. 

E-Mail

Engaging in email correspondence with patients medical-legally creates a patient-physician relationship devoid of privacy. Emails to patients are subject to informed consent prior to each electronic exchange. Security mechanisms have to be explained and the names of all persons with potential access to the email message have to be shared with the patient. Printing and storage of all patient-physician emails has to legally become part of the medical chart. 

Consequently, this office does not engage in email correspondence. All questions concerning diagnosis, treatment options and cost are discussed exclusively with your doctor in the office following a proper medical evaluation. 

Prescription Refills

If you need a prescription refill, please contact your pharmacist first, since you may still have refills which you are unaware of. Have the pharmacy contact us with the medication that needs to be refilled. We will have your prescription filled the same or next business day. 

If it has been more than a year since your last visit, you will need to schedule an appointment. We will not refill prescriptions for patients who do not keep appointments. Evaluating the effects and side effects of prescribed medications is our medical and legal responsibility. In addition, medical technology is always changing and you may be missing out on a more effective or safer medication. 

Scheduling Radiologic and Imaging Procedures

Donna Hagberg MD LLC will facilitate scheduling and precertification of non-routine imaging, such as diagnostic mammography, MRI, CAT scans and ultrasounds. However, appointments for routine annual or biannual screening mammography or bone density studies are the responsibility of the patient and need to be made directly with the radiology facility. 

Patient Records

Patient's records are the sole property of Donna Hagberg MD LLC.   Copies of these records will be released upon written request to the patient or to her designated medical practitioner. Charges legislated by the State of Connecticut for mailing and reproduction apply.
